Students are required to have a completed health form on file at the Wilkins Wellness Center prior to the start of classes.
All students attending at Shenandoah University(“SU”) are required to submit to the Wilkins Wellness Center (“WWC”) a fully completed Health and Insurance Requirements Form (the “health form”) and appropriate supporting documentation. Students in our nursing, pharmacy, or health professions schools, such as Music Therapy, Athletic Training and Occupational Therapy programs require immunizations and tests in addition to those required of students in other schools/programs.
All incoming students must have:
- Physical examination
- Two MMR (measles, mumps & rubella) vaccinations or a positive titer (blood test to prove immunity)
- Last date of the polio series or a positive titer
- Adult Tdap (tetanus, diphtheria & pertussis) within the past 10 years regardless of last Tdap vaccination
- Meningitis ACYW135
- Two or three meningitis B vaccinations (dates depends on manufacturer) or exemption form
Students in the Health Care Programs must have the above and the following:
- Two Varicella (chicken pox) vaccinations or a positive titer
- Covid-19 vaccinations and be up to date per CDC guidelines
- Three Hepatitis B vaccinations or a positive titer
- Current influenza vaccination after September 1 for current flu and academic year
- Two TB skins tests and two readings with the second test and reading 7-21 days from the first test, or a QuantiFERON TB Gold test within the last year.
- NOTE: Positive TB skin test from an exposure, positive QuantiFERON, latent TB or BCG vaccination, the radiology report for a chest x-ray within the last 12 months and Statement of Treatment Form (included) signed by a MD/DO, NP or PA. Radiological report and Statement of Treatment Form must be submitted with the health form.
All incoming Athletes:
- Sickle Cell Trait blood test Medic
Note: Commuter students, those not enrolled in a health program, or participating in a sport, are not required to have a physical examination; however, they must complete the remaining portions of the health form.
Students who fail to provide these documents before the deadline will have a $30 late fee charged to their student account and may have their admission revoked, or be suspended or dismissed from SU, and will not be allowed to attend classes, laboratory sessions, or clinical rotations.
Shenandoah University (“SU”) may grant a request for a medical or religious exemption to one or more of the above requirements. Students in the Health Care Program or any other SU program that requires clinical or experiential training, including music therapy (“Clinical/Experiential Programs”), should be aware that if SU grants the student’s request, it cannot guarantee SU will be able to secure the clinical/experiential assignments necessary for the student to graduate with a degree, or to obtain a licensed position, in their field. Health Professions and Pharmacy students have additional immunization requirements that must be met prior to starting classes.
All students are required to have health insurance. Students are required to register their insurance information online. You must upload a copy of your current insurance card both front and back at su.studenthealthportal.com.
Online/distant learning students are exempt. Online/distant learning students are not eligible to purchase this insurance plan.
To waive the Student Health Insurance plan:
- Log on to http://rcmdstudentbenefits.com/Shenandoah/
- Click on “Student Waiver or Enroll”
- Select with the RED “I want to Waive” button. If waiving the insurance, you’ll need information found on your current health insurance ID card. Upon completing your online form, you will be asked to review the information provided and click “submit” to complete the process.
The deadline to complete this online process is August 1.
Student health insurance is still the best option for college students. Even though some states give subsidies, the difference in the deductible alone makes student insurance the most affordable option. Medicaid programs only cover the student in their home state.
All students who are registered must be covered under a health insurance plan.
Online/distant learning students are exempt. Online/distant learning students are not eligible to purchase this insurance plan.
If you want to purchase the student health plan, follow the instructions below.
To enroll in the Student Health Insurance plan:
- Log on to student health insurance
- Click on “Student Waiver or Enroll” and follow the prompts
The deadline to complete this online process is August 1.
International Student insurance plans, those purchased outside of the United States, must meet the guidelines of the Affordable Care Act 2010 regulations to be an accepted plan.
- Please visit our insurance website at student health insurance and create a user name and password.
- Click waive the student health insurance button and input your health insurance information into the website.
RMCM&D will audit the insurance to see if the policy meets the guidelines. If the policy meets the guidelines, you are considered covered as long as your insurance is valid while enrolled at Shenandoah. If your plan does not meet the guidelines, you will have to purchase a plan that does meet the requirements or enroll in the student health insurance policy approved by Shenandoah University. Most travel insurance policies do not meet the guidelines.
In addition to the University’s Health and Insurance Requirement form all athletes must complete the Medical Participation Forms and submit it to the athletic department along with a copy of the physical examination page.
Students participating in sports are required to have a physical examination by their primary care provider.
You must notify the Athletic Trainer immediately if you have an injury while participating in sports. A completed claim form must be submitted within 90 days from the date of injury. You can visit the website at: www.eiia.org/shenandoah
Every full-time student who participates in Intercollegiate athletics is automatically enrolled in the Athletic Accident Plan. In the event of an accident, you must:
- Report the accident to your coach or athletic trainer immediately.
- File all charges with your primary insurance carrier first. If you are insured by an HMO/PPO, you must obtain pre-authorization for all services rendered or benefits will be reduced by 50%.
- If the other insurance does not pay the entire bill, secure a claim form and instructions from the Athletic Department, fill in information completely, have the attending Physician and supervising coach complete their portion of the form, attach all itemized medical and hospital bills and mail them to the claims administrator.
